High Needs Case Manager

Position Summary:

The Birth to Five Therapist provides developmentally appropriate, trauma-informed, and relationship-based therapeutic services to children ages 0?5 and their families. Utilizing evidence-based practices, the therapist supports social-emotional development, addresses behavioral and mental health concerns, and strengthens the caregiver-child relationship. This position works closely with behavior coaches to support developmentally appropriate interventionism home. This position requires a deep understanding of early childhood development, attachment theory, and the impact of trauma on young children and families.

Essential Responsibilities:

  • Provide individual, dyadic, and family therapy to children birth through age five and their caregivers.
  • Conduct comprehensive developmental and mental health assessments using age-appropriate tools.
  • Develop and implement individualized service plans (ISPs) in collaboration with caregivers and interdisciplinary team members.
  • Utilize trauma-informed and evidence-based models such as Child-Parent Psychotherapy (CPP), PCIT, TF-CBT (for young children), or other approved interventions.
  • Support caregiver engagement and enhance parenting capacity through psychoeducation and reflective practice.
  • Collaborate with pediatricians, early intervention providers, case managers, schools, and other community partners to support coordinated care.
  • Participate in Child and Family Team (CFT) meetings, Individualized Family Service Plan (IFSP) meetings, or other system meetings as appropriate.
  • Maintain accurate and timely documentation in accordance with agency, state, and health plan standards.
  • Participate in clinical supervision, peer consultation, and ongoing training in early childhood mental health best practices.
  • Attend agency and community meetings related to early childhood systems and services.

Required Qualifications:

  • Master's degree in Counseling, Social Work, Marriage and Family Therapy, Psychology, or related behavioral health field.
  • Licensed or license-eligible in the state of Arizona (LAC, LMSW, LAMFT, LPC, LCSW, LMFT).
  • At least one year of experience working with children ages 0?5 and their families.
  • Knowledge of infant/early childhood development, attachment, trauma, and family systems.
  • Strong interpersonal, organizational, and communication skills.
  • Ability to work independently as well as collaboratively in a team environment.
  • Valid driver's license and reliable transportation for home or community-based visits, if required.

Preferred Qualifications:

  • Bilingual in English and Spanish.

Position Summary:

A Birth through Five Specialist works to support early childhood caregivers, guardians, educators and families in promoting the healthy development of children from birth through age five. This role involves providing coaching, coordination of care, referrals, training, and resources to improve early childhood education practices, enhancing child outcomes, and support family engagement.

The Birth through Five Specialist is responsible for meeting their clients in childcare centers, preschools, and within the community, helping to support and promote the healthy development of children from birth through five. This role involves working with children, their families, caregivers, guardians and CT teams to create and implement support services for the families, caregivers and guardians.

Essential Functions (duties):

  • Develop and implement individualized behavior intervention plans, support plans education and interventions for assigned children and families, based on assessments, data analysis and goals created in the Child and Family Team.
  • Provide assistance to children and families by identifying strengths and needs, accessing support, and collaborating with other departments and also partnering with other professional agencies.
  • Provide a minimum of 30 hours of face-to-face direct support services to enrolled clients.
