Position Overview: We are seeking a Neurosequential Model of Therapy Clinician to join our dynamic team. The ideal candidate will have experience working in a diverse and multicultural environment. This role involves providing individual, family, and group therapy to clients of all ages, addressing a variety of mental health and behavioral issues.

Key Responsibilities:

  • Conducts a comprehensive NMT metric assessment and makes treatment recommendations, summarizes primary presenting issues, and identifies child/family strengths.
  • Reviews behavioral health documentation produced by BHT's and provides feedback to BHT staff when necessary.
  • Develops interim service plans that identify the next steps taken after intake by the provider and person/family, and what issues are to be addressed for ongoing assessment.
  • Provides effective crisis assessment including the following of program triage protocol, arranging for emergency psychiatric evaluation/hospitalization as indicated by protocol, provides for follow-up, normalizes situation, discusses options and handles situation calmly.

Qualifications:

  • Bachelor's degree in counseling, psychology, social work, or related field of study with 3 years of behavioral health experience
  • Master's degree preferred.
  • Preferred license or license eligible.
  • Must be NMT Phase I trained and have a minimum of an acceptable NMT Fidelity score.

Behavior Coach

Company Overview: Open Hearts Family Wellness provides Intensive in-home, community, and school-based support for individuals with behavioral challenges or Autism Spectrum Disorder. We provide evidence-based, person-centered services to promote positive behavioral changes. We are seeking a compassionate and motivated Behavior Coach to join our team and make a meaningful impact in the lives of those we serve.

Job Summary: The Behavior Coach will work with children and families to help them develop and implement strategies for positive behavioral change. The ideal candidate will be skilled in positive behavior support, goal setting, and motivational strategies to support children and families in overcoming challenges.

Essential Functions (duties):

  1. Develop individualized education and interventions for assigned children and families, based on goals created in the Child and Family Team.
  2. Provide assistance to children and families by identifying strengths and needs, accessing supports, and partnering with other professional agencies.
  3. Provide a minimum of 30 hours of direct support services to enrolled families per week.
  4. Assist and support the family in developing social and natural support networks by utilizing community resources.
  5. Introduce parents to alternative parenting techniques (positive behavioral support).
  6. Provide feedback to the Child and family Teams on progress and the interventions being utilized, or on barriers impeding progress.
  7. Provide transportation to child and family as determined by the family’s Child and Family Team and service plan.
  8. Provide crisis intervention and stabilization support as needed.
  9. Attend scheduled department and agency meetings.
  10. Attend trainings and supervisions, as assigned by supervisor.
  11. Assist in actively assessing work area for non-compliance issues and notify supervisor in a timely manner to ensure follow up.
  12. Complete all required documentation and billing requirements accurately and in the assigned time frames.

Education and Experience:

  • Associate’s degree with 2 years of experience working with children with a mental health diagnosis,
  • Bachelor’s degree or higher with 1 or more years of experience working with children with a mental health diagnosis,
  • Bachelor’s degree in a behavioral health related field (i.e., Psychology, family studies, human development, sociology, etc.) Some experience with mental health preferred
  • Previous experience as a behavior coach highly preferred
  • Bi-Lingual in Spanish preferred

Job Summary: Residential Support Staff provide direct care and supervision to residents in a Behavioral Health Residential Facility (BHRF). This role is responsible for ensuring the safety, well-being, and daily functioning of residents while promoting a supportive and therapeutic environment. Staff members implement individualized treatment plans, assist with daily activities, and foster positive behavior change.

Essential Functions (duties)

Resident Support and Supervision:

      • Provide direct supervision of residents in accordance with treatment plans and safety protocols.
      • Support residents with activities of daily living (ADLs), including meal preparation, hygiene, and medication reminders.
      • Monitor resident behavior and ensure compliance with facility rules and expectations.

Therapeutic Environment:

  • Build positive and therapeutic relationships with residents.
  • Facilitate group activities, skill-building exercises, and recreational programs.
  • De-escalate situations using crisis intervention techniques and follow facility protocols.

Documentation and Communication:

  • Maintain accurate and timely documentation of resident activities, behaviors, and incidents.
  • Communicate effectively with team members, supervisors, and external stakeholders, including case managers and clinicians.
  • Participate in shift change meetings to ensure continuity of care.

Safety and Compliance:

  • Conduct routine safety checks and ensure the environment remains safe and clean.

Training and Development:

  • Attend mandatory training sessions, including CPR, first aid, and crisis intervention.
